We are an "out-of-network" physical therapy provider. We automatically provide you with a coded receipt you can submit for reimbursement.
Have a health savings account?
If you have a Health Savings Account, you can use it as coverage for your Physical Therapy.

To view the benefits provided by your insurance coverage, you may need to register and login online first. We've provided courtesy links to some common insurance companies to help you with this step.

Fees
Initial evaluation (60 minutes): $220
Follow-up session (60 minutes): $208
Package of 4 follow up sessions:
- $789 ($197.25/session)

*Freelance professional dancers receive discounts on all services, $149 is charged per appointment.

Some people's goals benefit from meeting multiple times per week. Payment is due at the end of each session, either by check, cash, or card. A typical course of care is 4-12 visits.

Some dance companies (such as SFB, ODC, and Lines) are contracted with SF Performing Arts PT. Contact us if your company would like to establish a contract.

Worker's Compensation is accepted for dance companies only.
Cancellation Policy
​
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists’ day that could have been filled by another patient. As such, we require 48 hours notice for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice, or miss their appointment, will be charged a cancellation fee of $100.
